Job Summary

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment.

Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU Total Rewards of a Allied Health Professional Major Duties: Serves as the point of contact for the Healthcare for Homeless Veterans (HCHV) Program, responsible for answering referrals/consults from VA facilities and community resources. Responsible for assessing all Veterans that enter into the HCHV program.

This includes identifying all psychosocial needs of the Veteran and determining the appropriate services required. Along with completing treatment plans and clinical reminds/postings. Verifies the Veteran's status and eligibility for VA and non-VA resources, along with eligibility criteria for VA and non-VA programs. The social worker must have knowledge of VA and non-VA resources and the ability to work within multiple systems to advocate appropriately for Veterans.

Responsible for collecting and submitting HOMES assessments as indicated. Responsible for working with the local Continuum of Care (COC), COC subcommittees and Homeless Housing Task Forces committees, along with ensuring VA is in compliance with COC. Complies with criminal conflict of interest laws and Executive Branch Standards of conduct so as to avoid conflicts of interest while serving as the HUD-VASH Case Manager.

Works with the VA Medical Center multi-disciplinary teams and community agencies to strive for developing more effective and efficient ways of delivering healthcare services to homeless veterans. Participates in program planning and development, advising the supervisor of shifting trends with caseloads and identifying the need for and making recommendations on program procedure or policy changes based upon work experiences.

Takes responsibility for improving professional skills, participating in planning and executing specific staff development projects, attending seminars and institutes, and participating in training programs of other disciplines as requested. Provides a full range of case management services within commonly accepted standards of practice which includes case work, individual, family and group counseling, discharge planning, case management and other services as appropriate.

The social worker will assist patients and their significant others with coping and dealing with the loss and grief experiences in disability, terminal illness and death. Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
