This position will be located in the Social Work Service division within the Dayton VA Medical Center. Responsibilities include assessment of psychosocial problems and will help the patient and family to understand the contributing factors to the problem(s), discussing with them the pros and cons of possible short-term and long-term solutions.

Establishes a proactive, resourceful approach to discharge planning. This includes initiating, coordination, and expediting the successful discharge of Veteran clients. Participates effectively in team meetings, unit huddles, treatment planning conferences, and other teams and meetings as assigned. Documents in EHR in accordance with VHA Directive and policy.
