Job Summary

The Social Worker practices in the Healthcare for Homeless Veterans (HCHV) Program. The Housing Specialist functions as part of the Housing and Urban Development/VA Supportive Housing Program (HUD/VASH) within the HCHV Program. HUD/VASH is an intensive case management program serving chronically homeless and homeless Veterans aimed at providing psychosocial services to Veterans to obtain and maintain stable housing utilizing HUD's Section 8 voucher system.

Key Responsibilities

Providing education regarding the HUD/VASH Program, development and maintenance of housing resources to property owners/managers Educating Veterans regarding housing resources and assisting with application process Assisting and accompanying Veterans in locating appropriate housing and preparing for housing interviews Working closely with HUD/VASH case managers regarding admission to HUD/VASH program, obtaining housing and housing retention efforts, as well as transporting Veterans for housing searches Performing case finding functions and outreach, identifying and screening patients for psychosocial needs Conducting psychosocial assessments Developing treatment plans in collaboration with the veteran/family and with the interdisciplinary treatment team Coordinating community-based services, including information and referral for additional services from other VA, government, and community agency programs Identifies high-risk patients and providing crisis intervention services Working with patients and families who are experiencing a variety of psychiatric, medical, and social problems utilizing individual, group and family counseling and therapy skills Providing consultation services to other staff regarding the psychosocial needs of veterans and the impact of psychosocial problems on health care and adherence to treatment plan Work Schedule: Monday - Friday, 8:00 am to 4:30 pm.
