The incumbent will serve as a Social Worker within the Social Work Service at the Louis Stokes Cleveland VA Medical Center based in the Community Residential Care program.

Developing a resource file of VA and community social service programs and will refer the veteran to needed services; and provides assistance if the veteran is unable due to cognitive and/or medical/physical deficits. Perform screenings for appropriate referrals to the Community Residential Care program in conjunction with team members. Completes clinical reminders and provides education related to VA and community resources, State and Federal entitlements, and will refer Veterans/families tot he appropriate interdisciplinary team member for identified healthcare and patient education needs and resources. Develops, implements, and and regularly updates individualized treatment plans based on assessments, in collaboration with the Veteran/family and the interdisciplinary treatment team, focusing on Veteran's stabilization, maintenance and recovery goals.
