Job Summary

The Primary Care Licensed Vocational Nurse (LVN) provides high-quality, evidence-based care for a culturally diverse Veteran population; emphasizing patient centered care to veteran's ages 18 to 75 and above in the outpatient care setting. They must be able to promote a safe environment for all patients including those with altered sensorium due to medication or an acute or chronic illness. The LVN will perform assignments at the ACC Bravo Clinic.

Key Responsibilities

The Licensed Vocational Nurse performs assignments in the Primary Care Clinic (Bravo Clinic) as a member of the Patient Aligned Care Team (PACT). Under the direction of the Registered Nurse or Nurse Manager, she/he is responsible for providing direct and/or indirect patient care to an assigned group of patients. This care takes into consideration all physical, psychosocial, spiritual and cultural needs of this specific Veteran population.

The LVN delivers safe and timely care framed by evidence-based practices and a true caring for the Veteran population and their significant others. - Collects data and documents findings on newly assigned/ established PACT patients including common observations and actions in electronic health record Computerized Patient Recording System (CPRS) and appropriate forms if indicated. - Administers medications per Nursing Service Policy and Scope of license.

- Ability in performing EKG.- Able to recognize patients in distress needing immediate intervention. - Recognizes the signs of potential or active disruptive behavior and quickly intervenes with appropriate therapeutic interventions, including activates Crisis Action Team (CAT). - Demonstrates ability to use "Safe Patient Handling" Equipment and techniques, able to transport, turn and move patients using proper equipment.

- Gathers and records data reflective of the patient's (and/or families') physical, psychosocial, emotional, spiritual, cultural, and educational needs. - Recognizes and appropriately responds to breakage/malfunction or loss of equipment, safety hazards, and supply deficiencies, promptly reporting to appropriate personnel for corrective action.-Documents in a timely, accurate, and concise manner.

- Identifies common changes in condition, both physical and emotional, which have relevance to health status. - Participates in planning the nursing care provided to patients with a variety of physical and/or behavior problems.- Provides patient-related data. - Prepares, administers and documents medications with alertness to actions, side effects, adverse reactions and contraindication following Medical Center policies/procedures.
