Job Summary

The Register Nurse for Women's Health (WH) Patient Aligned Care Team (PACT) Outpatient is responsible and accountable for individual nursing practice and determines the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal care. This nurse provides direct gender-specific Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department.

Key Responsibilities

Major duties include but not limited to: Completes orientation according to expected standards. Works with close supervision, is responsible and accountable for individual nursing practice and seeks direction from others as needed. Manages workload as assigned, organizes, and completes own assignments in an efficient and appropriate manner. Participates in the development, implementation, and evaluation of interdisciplinary care.

Establishes a therapeutic relationship, allowing the patient to attain, maintain or regain optimal function through assessment and treatment. Engages patients, families, and other caregivers to incorporate knowledge, values, and beliefs into care planning without judgement or discrimination. Knowledgeable of ethical issues related to professional nursing practice and follows established policies of the practice setting, VA, and ANA Code of Ethics for Nurses.

Aware of high reliability principles to deliver consistent care and improve patient outcomes. Participates in unit based educational activities and continuing education requirements. Responsible for maintaining competency to continue personal and professional growth. Applies evidence-based practice/research to patient care. Participates in unit-based activities to improve and deliver cost effective patient care.

Demonstrates knowledge of specific unit level performance improvement activities. Incorporates patient preferences into shared care delivery decisions. Leads and organizes delivery of care to ensure continuity of care and peer accountabilityfor practice in alignment with the ANA Standards of Nursing Practice. Applies advanced clinical knowledge/judgment to lead staff involvement in planning,decision-making, and evaluating outcomes beyond the immediate practice setting.

Applies nursing expertise to lead improvement, with demonstrated outcomes, beyondthe immediate practice setting. Leads initiatives to improve the Veteran experience i.e., care coordination, timeliness toresolve issues, negative trends, or environment of care concerns, beyond the immediatepractice setting. Demonstrates leadership in patient advocacy by protecting the health, safety, and rightsof patient population groups beyond the immediate practice setting.

Leads staff to identify, analyze and address ethical issues that impact patients,caregivers and staff beyond the immediate practice setting. Demonstrates positive, effective communication skills, professional behaviors andleadership that promotes a commitment to excellence, improving nursing practice, andfostering collaboration among customers beyond the immediate practice setting.

Makes sustained contributions to healthcare by actively participating in healthcarefacility or service-level committees or councils, establishing active relationships anddemonstrating leadership in improving care beyond the immediate practice setting.

Requirements & Education

Note: Only education or degrees recognized by the Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE) may be used to qualify for nursing positions.

The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration.
