Job Summary

The Advanced Practice Nurse (APN) II Home Based Primary Care (HBPC) Nurse Practitioner (NP) practices in the HBPC program under the Geriatrics & Extended Care (GEC) service line to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. The NP provides advanced clinical practice, consultation, and management.

Key Responsibilities

The NP practices to the full extent of their education, training, and certification. Core functions are outlined in the clinical privileges that correspond with this position. The NP serves as a core member of the HBPC interdisciplinary team as primary care provider with prescriptive authority or a panel of patients in the HBPC program. The NP functions independently with as needed guidance from the supervisor.

HBPC program operates under the guidance of HBPC medical director and collaboration with HBPC program director. Major duties and responsibilities include, but are not limited to: Manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice. Develops, implements, and modifies an individualized plan to achieve optimal population health and disease management.

Demonstrates leadership and works collaboratively with HBPC interdisciplinary team members. Orders and interprets labs, tests, and diagnostic studies and accurately communicates results and management plan in a timely manner. Prescribes appropriate pharmacologic and non-pharmacologic interventions. Initiates referrals and consults to specialists and community care services in a timely manner. Assesses learning needs and provides education to patients and their families.

Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes using shared decision making. Exemplifies the ICARE core values of-Integrity, Commitment, Advocacy, Respect, and Excellence. Coordinates timely admission to, transfer from, and discharge of patients in the HBPC program, ensuring appropriate transition of care. Develops protocols and clinical pathways to standardize care and enhance and expedite care delivery in HBPC.

Performs comprehensive patient assessments including the use of telephone and video technology for patient evaluations as indicated. Triages patient, family, or agency telephone calls for patient care issues of assigned patients for timely intervention including early access to clinicians as appropriate. Participates in program quality management, quality improvement, and/or process improvement.

The NP will participate in service-level recruitment, peer support and guidance, and role transition including new HBPC employee orientation. Provides after hours call coverage for HBPC as needed.
