Job Summary

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application.

Former EDRP participants are ineligible.

Key Responsibilities

The Clinical Pharmacy Practitioner (CPP) Supervisor, Primary Care/Outpatient Clinical Pharmacy Programs functions at the highest level of clinical practice, working independently under his/her scope of practice as defined by the medical center to directly care for patients requiring care as deemed necessary and appropriate in his/her designated area of specialty at the parent facility and to other areas of the medical center as assigned.

The CPP Supervisor, Primary Care/Outpatient Clinical Pharmacy Programs, is responsible for the professional and administrative management of an assigned area in pharmacy service, plays a defined role in budgetary execution, and serves as a consultant and midlevel provider who functions to initiate, modify or discontinue medication therapy. Extensive documentation and clinical progress note writing are required of this position.

The incumbent may be involved in research activities and precepting of pharmacy trainees. VA Careers - Pharmacy: https://youtube.com/embed/Fn_ickNBEws Serves as a provider with prescribing privileges as outlined in the individual's Scope of Practice to perform comprehensive medication management (CMM) and anticoagulation management. Develops policies and procedures relative to the assigned area of practice.

Designs and implements orientation and training programs for staff and develops and maintains systems to monitor the performance of staff activities. Supervise Pharmacy Residency Program and other educational programs if applicable to his/her area of practice. Supports the Chief of Pharmacy as needed during various inspections and surveys such as Joint Commission.

Responsible for tracking and trending workload within clinical area of supervised clinical practice through proper clinic set up and proper use of encounter coding. Incumbent will monitor timely encounter completion, signing/cosigning of progress notes and consults open for the outpatient clinical pharmacy programs. Evaluates Focused Professional Practice Evaluation (FPPE) and Ongoing Professional Practice Evaluation (OPPE) for scope of practice (SOP).

S/he will assist with professional practice evaluations for CPP assigned to the Outpatient Clinical Pharmacy Programs and maintenance of scopes of practice for CPP assigned to the Outpatient Pharmacy Programs. Provides input in the revision of performance plans, issuing performance plans, tracking and documenting employee performance, making recommendations on performance ratings to the Associate Chief of Pharmacy, Clinical and Educational Programs for consideration.

Coordinates all clinical pharmacy functions for his/her assigned practice area, helping to achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams. Monitors and assesses the outcome of drug therapies with advanced skill, including physical assessment and interpretation of laboratory and other diagnostic procedures.
