Duties and responsibilities of the Interventional and Specialty Care Coordinator include, but are not limited to: Care Coordinator Registered Nurse (RN) provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings. The Care Coordinator assists Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs.
This includes, but is not limited to, acute, chronic, multiple, complex, catastrophic, or life-threatening illnesses; combat stress, residuals of traumatic brain injury; community adjustment; addictions and other health problems. Coordinates care with multiple providers across all levels and sites of care. Coordination also includes assuring services interface within the Veterans Affairs (VA) and community programs to provide continuity of care to patients.
The Care Coordinator addresses psychosocial, as well as nursing and medical needs of patients and their families/caregivers, through participation with the interdisciplinary care team. Coordinates and expedites care from diagnosis to treatment. Analyzes and manages laboratory and radiology results and provides information to the providers. Utilizes databases to provide population management to track patient care through all episodes of care.
Evaluates care and outcomes to ensure timely and appropriate provision of services. Advocates for holistic care through patient/family education and provider interaction and implements an educational plan to meet the patient care needs. Administer medications and treatments according to policy. Monitor patient responses and promptly report changes. Provide competent, evidence based care to patients. Coordinates care with multiple providers across all levels and sites of care.
Medical device equipment uploads to patient charts if applicable. Analyzes and manages laboratory and radiology results and provides information to the providers. Utilizes databases to provide population management to track patient care through all episodes of care. Review scrubbing/charts and verify medication, lab and imaging orders. Conduct preliminary diagnostic workups. Ensure completion of required tests (e.g., chest X-rays, EKGs, labs) prior to appointments.
Medical Device Equipment Uploads to Patient Charts if applicable. Complete Goals of Care with patient/family.Collaboration with Community Care and social Services. Referrals to appropriate services within scope of practice(Dietician, Whole Health etc..) Participate in educational events and complete required courses by deadlines. Perform additional duties as assigned by the Nurse Manager or designee.
Follow standard workflows and perform functions within scope of practice as assigned.
Engage in care coordination activities to improve efficiency and patient outcomes, including: Enhanced patient satisfaction and education Reduced complications and readmissions Lower fall risk Improved medication adherence Optimal symptom management Decrease Community Care Outsource Responsible for participating within unit based and/or organizational level Shared Governance councils, utilizing clinical expertise, evidence-based practice, and knowledge to assist with finding solutions to problems or issues to improve the quality care provided to our veterans.
