Job Summary

The Licensed Vocational Nurse (LVN) position supports the Patient Aligned Care Team (PACT). The LVN provides comprehensive practical nursing care, including treatments, procedures, and patient education. Duties involve rooming patients, taking vitals, administering medications, assisting with minor procedures, and documenting in electronic health records. The role requires strong clinical skills, teamwork, and the ability to handle a high volume of patients.

Key Responsibilities

Total Rewards of a Allied Health Professional The LVN demonstrates the ability to balance many competing demands and sets priorities effectively, while supporting multiple providers. Has the knowledge and ability to appropriately carry-out assigned patient care based on patients' conditions. Demonstrates competency in dealing with a high volume of patients with chronic disease.

Completed work requires a general review by Registered Nurse for appropriateness and conformity with established policies and procedures.

Duties include but are not limited to the following: Direct Patient Care: Has knowledge and skill in performing support duties for complex diagnostic tests and/or specialized practices or procedures Provides quality care to patients with multi-system problems, including acutely- and chronically ill patients Promptly recognizes emergent patient care situations and/or subtle changes in patient status and reports to RN or provider.

Demonstrate self-direction in prioritizing and completing assignments Assists PACT teams with post discharge calls, health coaching, and patient goal setting Observes, identifies, and responds to patients' needs for care, including medication and equipment-assisted care, and recognizes deviations from normal Appropriately performs, reports, and documents assigned nursing care in the Primary Care Clinics Indirect Patient Care: Accurately recognizes, reports, and records relevant patient information Actively seeks educational opportunities to enhance nursing knowledge and skills Adheres to principles of infection control Assists in promoting safe work habits and is a role model for other staff in fostering safety principles and practices Serves as preceptor in orienting, teaching, and training less experienced LVNs and NAs/HTs Work Schedule: Monday- Friday 8:30am - 5:00pm Recruitment/Relocation Incentives (Sign-on Bonus): Not Authorized Permanent Change of Station (Relocation Assistance): Not Authorized Pay: Competitive salary and regular salary increases.

When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).

Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience. Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.

Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.

Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement) Telework: Not Available Virtual: This is not a virtual position.
