This position is located at the Lawton Indian Hospital. Incumbent is responsible for all aspects of Case Management for all patients. Coordinates available resources by identifying, organizing, and utilizing resources to meet the patient's needs. This position is supervised by the Supervisory Nurse (Case Manager).
