This position is located in the Division of Nursing, Outpatient Departments, Whiteriver Service Unit, Whiteriver, AZ. The incumbent is member of primary medical home care team with both medical provider staff and other health care staff (clinical and non-clinical). A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021).
Responsible for setting up and monitoring patients enrolled in disease specific home monitoring and communicating clinical information of changes to clinical plan between patient and provider/care team. Develop and maintain patient registries based on disease, health condition and/or health prevention initiatives. Assist with triage of patients, including triaging telephone calls and walk-in patients. Prepare patients for tests, exams and treatments.
Coordinate appointments and procedures for patients who need further care or continuity of care at other facilities. Provide counseling and teaching to patient and families on preventive, curative and rehabilitative measures involved in health care.
