The Nurse Manager - Community Care is primarily responsible for coordinating and implementing the Community Care program for Veterans. This role ensures high-quality nursing practices in the referral, implementation and monitoring of patient care as Veterans move through the program.
The Nurse Manager uses analytical frameworks, such as the nursing process, to create an environment that supports the delivery of non-medical care & collaborates with multiple disciplines and community vendors.
Major Duties: Coordinates care for Veterans referred to community care, ensuring eligibility and proper documentation. Provides leadership and supervision to nursing staff, including scheduling, performance evaluations, and disciplinary actions. Ensures compliance with standards of care, regulatory requirements, and organizational policies. Manages clinical and administrative issues such as staffing, leave, and patient or staff concerns.
Facilitates partnerships with physicians, clinicians, and community vendors. Participates in policy development and quality improvement initiatives. Maintains a focus on patient safety, satisfaction, and positive outcomes.
