Job Summary

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application.

Key Responsibilities

This position is located at the McMinnville Outpatient Clinic in McMinnville, TN. The NP is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients.

Major Duties: Independently and responsibly manages a panel of position: patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice Develops and implements an individualized plan to achieve optimal population health and disease management Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members Performs histories and physical exams Provides appropriate documentation to minimally include history, physical exam findings, diagnosis, definitive care plan and disposition of the patient Ensure that patient encounters are appropriately closed out including disposition that clearly indicates the next step of care Ensure that each patient is provided with a list of current medications and an appointment list at close-out Communicates test results to patients Orders and interprets labs, tests, and diagnostic studies Prescribes pharmacologic and non-pharmacologic interventions Initiates referrals and consults to specialty services Provides education to patients and their families Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes Exemplifies the ICARE core values of-Integrity, Commitment, Advocacy, Respect, and Excellence Demonstrated experience leading clinical quality improvement initiatives or managing healthcare programs.

Background in project management within a clinical setting strongly preferred. Training or certification in quality improvement methodologies (e.g., Lean, Six Sigma) is a plus.
