The TeleOncology Care Coordinator staff nurse is a registered nurse responsible for the highly specialized care of the cancer patient through the continuum of care: diagnosis, treatment, and follow-up care.
Responsible for providing competent, evidence-based care to oncology and hematology patients Provides patient management, care coordination, and discharge/disposition planning for the outpatient setting Coordinates care with multiple providers across all levels and sites of care Collects and tracks data specific to the diagnosis, diagnosis-specific work-up, treatment plan, and treatment experience of the patient with cancer Analyzes assessment data to determine the actual or potential patient care needs, or treatment team needs to care for the patient.
Individualizes plans of care for the patient and caregiver support focusing on health promotion and maintenance, symptom management, rehabilitation, or a comfortable death Employs strategies to engage and empower patients to promote health and a safe environment at all phases in the cancer continuum from prevention through the end of life Collaborates with the patient and caregivers, the interprofessional team, and community resources in the conduct of nursing practice Seeks new knowledge that reflects the current state of hematology and oncology nursing and cancer care and promotes critical thinking and innovation.
Participates in lifelong learning to maintain expertise in hematology and oncology nursing, disease-specific knowledge, regulatory standards guiding practice Must be a self-starter who is independent and flexible in practice. National TeleOncology is virtual service across 120+ VA sites of care and operates in 2 medical record systems.
