The VA Northeast Ohio Healthcare System is recruiting for an Registered Nurse (RN). The Registered Nurse (RN) will function in the Access Management & Care Coordination (AM&CC) section of the Medical Center.

The Nurse Coordinator duties include, but are not limited to: The Nurse Coordinator treats all patients, families and other caregivers with dignity and respect incorporating knowledge, values, and beliefs into nursing care delivery without judgement or discrimination.
Leads and organizes delivery of care to ensure continuity of care and peer accountability for practice in alignment with the ANA Standards of Nursing Practice. Applies clinical knowledge/judgment to lead staff involvement in planning, decision making, and evaluating outcomes within the immediate practice setting.
Establishes a therapeutic relationship, allowing the patient to achieve optimal function through assessment and treatment of their health enhancing the patient experience, satisfaction, and/or safety. Applies nursing expertise to lead improvement, with demonstrated outcomes, beyond the immediate practice setting.
