The Marion Il VMAC is seeking a full-time registered nurse to join the Vincennes CBOC Primary Care Team.
As a Registered Nurse (RN) Case Manager, you will provide comprehensive, patient-centered care management services to support Veterans with chronic, high-risk, or post-acute conditions. You will conduct detailed nursing assessments, develop individualized care plans, implement evidence-based interventions, and evaluate patient outcomes across the continuum of care.
The RN triages patient-generated health information-including vital signs, symptom reports, and structured clinical data-to identify potential complications and initiate timely, clinically appropriate interventions. You will collaborate with an interdisciplinary team that includes PACT, HBPC, Mental Health, Social Work, Pharmacy, and Nutrition, and coordinate with community providers when shared care is required.
The RN delivers health education, coaching, and motivational support to enhance Veterans' self-management skills, reduce preventable hospitalizations, and improve access to care. The RN is accountable for adhering to all organizational policies, clinical guidelines, and regulatory standards while demonstrating sound clinical judgment, professionalism, and emerging leadership skills in providing safe, effective telehealth and outpatient nursing care.
Major Duties and Responsibilities Assist with nursing education, orientation, competency development, and support quality-improvement and outcomes-based initiatives. Demonstrate leadership in delivering and continuously improving holistic outpatient care through effective collaboration with interdisciplinary partners. Evaluate nursing practices on an ongoing basis using best evidence and clinical standards.
Provide peers with constructive, informal feedback to promote continuous improvement. Foster a safe, respectful, and supportive environment that encourages professional development for all team members. Contribute a professional nursing perspective to interdisciplinary team discussions and care-planning activities. Partner with colleagues and stakeholders to implement change and achieve optimal patient outcomes.
Support colleagues through knowledge-sharing and application of evidence-based practices to ensure safe, high-quality outpatient nursing care. Participate in activities and initiatives designed to sustain and advance evidence-based practice. Perform other duties as assigned.
