This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application.

The Clinical Pharmacy Practitioner (CPP), Primary Care, utilizes expertise in the area of Primary Care to provide medication management to optimize patient care. As an advanced practitioner of Primary Care he/she is authorized within the guidelines of a scope of practice to function as a provider of services to patients of the VA TVHS requiring minimal supervision in both the practice area of Primary Care as well as the specialty practice area in which he/she is assigned.
VA Careers - Pharmacy: https://youtube.com/embed/Fn_ickNBEws Duties include but not limited to: Functions as an advanced practice provider for CMM services to achieve defined outcomes through direct interactions with patients and providers in assigned care area(s). Interfaces with providers, including physicians, PAs, ARNPs in the design of the Veteran pharmacotherapy regimens to achieve optimization of rational and cost-effective drug therapy.
Prescribes, monitors, and adjusts medications related to Primary Care pharmacotherapy management. Obtains pertinent medical history and obtains accurate medication related history and family history from patient and via chart review. Obtains oral informed consent and documents this encounter in the electronic medical record when appropriate. Provides patient and/or caregiver instructions and education.
Utilizes computer expertise in interpretation of national dashboard analytics or development of local data extraction tools to help guide evidenced based prescribing for chosen disease states. Evaluates the drug literature by analysis of experimental design and methodology, in order to compare and contrast therapeutic regimens and roles for new drugs, with a special emphasis on medications used in Primary Care.
Assists in the use of data tools to promote the design and documentation of therapeutic regimens or plans, based on patient-specific information, which includes establishing a therapeutic endpoint and stop criteria, electing an appropriate drug and related therapy, and establishing an appropriate dosage regimen. Provides and promotes cost effective pharmacy services, offering reasonable, appropriate alternatives to high-cost drugs.
Accepts urgent or same day patient visits including, but not limited to, patient medication review for polypharmacy, recent hospital or emergency department discharges, and co-managed care patients. Collaborates with other TVHS staff, educating the Veterans and caregivers regarding the proper use of medications. Work Schedule: Full-Time, Based on the needs of the service.
