Job Summary

Surgery Care Coordinator (SCC) Registered Nurse (RN) functions directly to support the activities of the St. Cloud VA Health Care System Ambulatory Surgery Center (ASC). The RN is responsible for the planning, integrating, implementing, modifying, and evaluating the effectiveness of the Veteran's care. He/she is responsible for providing clinical leadership and expertise on Surgery and Specialty Care Integrated Clinical Community processes and regulations.

Key Responsibilities

Major duties include but are not limited to: Demonstrates flexibility, critical decision-making and collaborative skills necessary to coordinate surgery schedule. Coordinates master surgery schedule. Allocates surgery dates/numbers of rooms based on the block scheduling. Evaluates requests for surgery to ensure realistic numbers/length of surgical procedures to coordinate allocation of time and space by services.

Alerts appropriate contacts about potential need for implants/prosthesis prior to day of surgery. Able to transcribe surgery requests in the event of computer downtime. Screens surgical procedures against existing clinical privileges and surgical complexity. Inputs data into Vista Surgical Software Package and Surgical Access Database.

Case manages appropriate resources and tools to document, track, and assure follow through on patients with high risk needs or care management coordination. Conducts a comprehensive assessment of the patient's health and psychosocial needs, including health literacy status and deficits, and develops a plan collaboratively with the patient and family or caregiver.

Case manages with patient, family or caregiver, the specialist, the PACT team, other health care providers, and the community to maximize health care responses, quality, and cost-effective outcomes. Facilitates communication and coordination between members of the health care team, involving the patient in the decision-making process in order to minimize fragmentation in services.

Educate the patient, the family or caregiver, and members of the health care delivery team about treatment options (including but not limited to CVT, HT, evidenced based protocols), community resources, psychosocial concerns, and case management, so that timely and informed decisions can be made. Empower the patient to problem-solve by exploring options of care and alternative plans to achieve desired outcomes.

Encourage the appropriate use of health care services and strives to improve quality of care and maintain cost effectiveness on a case-by-case basis. Assist the patient in the safe transitioning of care to the next most appropriate level.
