The Clinical Resource Hub (CRH) is a strategic program designed to provide Team Based Primary Care, Specialty Mental Health and other specialty services in each VISN across the Enterprise, utilizing existing and emerging technologies. Telehealth technology improves Veterans' access to health care by matching clinical provider supply with demand, especially in rural areas where it is difficult to recruit.
The CRH Nurse Manager (NM) is the designated first-line supervisory authority and is responsible and accountable for the operation of their assigned units and provides coverage for other CRH NMs as needed. Major duties include, but are not limited to: Responsible and accountable for the direction and supervision of specific orientation, training, development of staff, and evaluation of individual staff performance.
Function as a role model, educator, practitioner, and supervisor providing leadership to the team. Independently provides professional assessment, coordination, telephone triage, and planning of multiple health care services. Collaborates with other members of the expanded health care team, facilitates coordination and transition of care between services, and may involve the patient and family in the decision-making process.
Minimizes fragmentation of healthcare delivery and optimizes quality of patient and family outcomes in a cost-effective manner. Supports the location to and from which care is delivered, including but not limited to Veterans Affairs (VA) clinical locations and provider alternate worksites. Supports the VISN CRH which may provide virtual and face-to-face services not only to clinical areas within the VISN CRH but also other VISN CRHs throughout the nation.
Services provided by CRH staff are conducted via multiple modalities, including virtual and in-person care. Responsible for collaborating with services internal and external to the VA to facilitate care transition to effectively meet the patients' needs.
