Job Summary

The Community Living Center RN is Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements, and evaluates care based on age specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines.

Key Responsibilities

The Community Living Center (CLC) Registered Nurse (RN) executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex resident care. Collaborates in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines. Demonstrates performance and leadership that is broad enough to improve the care for a group of residents.

Provides peers with formal or informal constructive feedback for improvement. Supports colleagues and other nurses through knowledge sharing to provide safe, quality nursing care. Fosters a safe and supportive environment conducive to the professional development of health care professionals. Evaluates outcomes of evidence-based decisions and practice changes for individuals, groups, and populations. Responsible for the documented outcomes at the program or service level.

The CLC RN collaborates with the Minimum Data Set (MDS) coordinator to ensure accuracy of the Resource Utilization Group (RUG) score. The responsibilities of the Registered Nurse (RN) with the competency to care for patients within the Community Living Center include, but are not limited to: Has oversight for admission/approval process for nursing home patients.

Responsible for providing Nursing Care to Long Term care residents, dementia unit residents, respite patients, hospice patients, rehabilitation patients, transitional care patients and their families/significant others. Have knowledge and expertise in: Pain management Palliative Care RAI/MDS Accu-max software Meds and Rx via BCMA Geriatric Assessment Skills Care of Dementia Patients Physical Restraint Policy & Procedure Discharge Planning and Teaching Accutech Pt.

Wanderer Security System Facilitate Inter-disciplinary Meetings End of Life Care Planning Rehabilitative Nursing Interacting with other therapy modalities, i.e., RT, PT, OT, Restorative Rx. Team building Safety Collaboration This clinical position includes a level of physicality normally associated with direct patient care duties performed by nurses such as potentially long periods of continued walking, standing, sitting, moderate lifting, repeated bending, or kneeling.

Permanent Change of Station (PCS): Not Authorized Physical Requirements: Standing (up to 2 hours), walking (up to 6 hours), stooping, bending, reaching, lifting, and turning; repositioning patients, assisting with transfers; pushing or lifting in excess of 45 pounds; visual (corrected to 20/30 or better with color discrimination); auditory acuity; arms above shoulder level; neck in a fully flexed or extended position; work on computers for extended periods of time.

Work is primarily conducted in a well-ventilated, adequately heated and cooled building. May be exposed to inclement weather when traveling around medical center. May be exposed to normal office irritants - dust, pollen, noise, vibrations, or odors. May be exposed to therapy animals with and without advanced notice. May be exposed to patients and family in emotional upheaval or extreme pain. Must have emotional stability to properly care for usual patient population.
