The Nurse Program Manager (PM) Registered Nurse (RN) is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Follows procedures per established policies and guidelines.

Also, demonstrates performance and leadership that is broad enough to impact the standard of care and safety of staff while caring for all patients throughout the continuum of care.
The Nurse Program Manager (PM) Registered Nurse (RN) is responsible for leading a facility wide program for all services within the organization that meets the requirements of National Directives. Other duties included but not limited to the following: Demonstrates leadership in delivering and improving holistic care through collaborative strategies with others. Review and address patient/caregiver/family concerns, problems, compliments.
Monitors, tracks, and reports program census, team, provider, and nurse panel numbers. Communication liaison between various services who make up the HBPC team. Monitor staff visits (Home, Telephone, and VVC) for regular frequency and annuals. Monitor and update Event Capture database for staff travel time, productivity monitoring. Receive, assign, and monitor HBPC consults for scheduled visits and required timelines.
Coordinate requests of new staff clinics, grids, PCMM updates. Maintain updated HBPC program staff lists for all locations. Collaborate with QSV for Joint Commission requirements, readiness, and staff training. Monitor and report all program quality data. Perform supervisory visits with HBPC staff. Collaborate with the HBPC Nurse Manager and Medical Director for program growth planning. Evaluates practice in an ongoing process, based on best evidence.
Fosters a safe and supportive environment conducive to the professional development of healthcare professionals. Contributes professional nursing perspective in discussions with the interdisciplinary team. Oversee the provision of staff education, orientation, competencies and providing quality improvement consultation related to the program. Questions clinical practices for the purpose of providing evidence-based care.
Participates in activities and strategies to sustain evidence-based-practice. Continuously evaluates the program, collecting and submitting facility performance measures. Program productivity tracking using data from Vista PCE report and Excel spreadsheet tracking. Assist with request for staff medical equipment, visit bags, tools, vital signs equipment. Monitor HBPC vehicle fleet for needs, milage, service, tires, accidents & repairs coordination.
Schedule HBPC staff meetings, agenda and minutes. Coordinate IDT meeting schedules and minutes at CBOC locations. Coordinate program policies in collaboration with QSV, Nurse Manager, and Medical Director. Monitor and report Patient Satisfaction Survey results. Monitor and report Infection Control hand hygiene monitoring. Monitor and report provider medication reconciliation. Collaborate with the VHSO VERA coordinator, monitor and report program VERA statistics.
Coordinate and update the HBPC emergency plan folders at all team locations. Coordinate and report for vulnerable veteran population emergency contact/response. Coordinate HBPC falls team quarterly meetings, monitor minutes and improvements goals. Coordinate HBPC quarterly chart reviews and quality monitoring reporting. Consistently communicates and treats customers (patients, visitors, volunteers, and all Medical Center staff) in a courteous, tactful, and respectful manner.
