The primary purpose of the Home Based Primary Care Nurse is to demonstrate leadership in delivering and improving holistic care through collaboration strategies with others in the assigned area to provide complex care to the vulnerable veteran at home.
Major Duties include, but are not limited to: Serves patients requiring inpatient or outpatient procedures for diagnosis and treatment of veterans with multiple comorbidities. Communicates with the physician as well as the interdisciplinary team on an ongoing basis concerning the patients condition and ongoing progress. Consistently assesses the need for necessary supplies in the home.
Provides care in a non-judgmental, non-discriminatory manner that is sensitive to the adult or elderly patient and family's lifestyle, preserving their autonomy, dignity and rights. Focuses on problems, initiates referrals to other disciplines and/or services as appropriate. Accurately documents in the medical record patient findings, assessments, and care provided. Effectively functions independently as contributing member of an interdisciplinary team.
Demonstrates and/or verbalizes correct responses to emergencies as well as initiates/appropriate action in emergencies. Provides education to the veteran and caregiver about developing an individual emergency preparedness plan. Initiates and utilizes positive, effective communication skills, and professional behaviors that promote cooperation, teamwork and effective interaction with internal and external customers.
Provides competent, evidence-based care to assigned patients and oversight of other disciplines as needed.
