Job Summary

The Registered Nurse will assume responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.

https://youtu.be/EGW4PMZvJto?si=_f7pNchVqoK1KZal Duties and responsibilities include, but are not limited to, the following: Focus on individual growth and development in practice with increased levels of self-direction. Provide patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings.

Assist Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs - This includes, but is not limited to, acute, chronic, multiple, complex, catastrophic, or life-threatening illnesses; combat stress, residuals of traumatic brain injury; community adjustment; additions and other health problems. Coordinate care with multiples providers across all levels and sites of care.

Develop a plan of care using information from the patient, family, and provider. Bridge the gap between discharge and primary care follow-up. Identify high-risk patient population and transitions to appropriate discipline (i.e., Primary Care, Home Based Primary Care, Hospice/Palliative Care, etc.). Responsible for intensive monitoring, follow up, and clinical management of high-risk patients.

Collaborate with interdisciplinary team members to develop and implement the Veteran Housing stability plan. Contribute to the team-based assessment of the ability of individual Veterans to live independently in the community, as part of eligibility determination for Veterans with complex medical and psychological risk factors.

Provide recovery-oriented and Housing First services, with the goal of establishing the Veteran independently in the community, at the Veteran's highest level of functioning. Address psychosocial, as well as nursing and medical needs of patients and their families/caregivers, through participation in interdisciplinary patient care management practice. Evaluate care and outcomes to ensure timely and appropriate provision of services.

Bring principles and theory with limited application of critical thinking skills.
