Job Summary

The Maternity Care Coordinator RN (MCC RN) delivers fundamental knowledge-based care to assigned clients while developing technical skills. The MCC RN assesses, plans, implements, and evaluates care based on age-specific components. The MCC RN assumes responsibility for coordinating care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.

Key Responsibilities

The Nurse II Maternity Care Coordinator Registered Nurse (MCC RN) is responsible for providing competent, evidence-based care to assigned patients. Provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings. The Care Coordinator assists Veterans and/or CHAMPVA clients, family members, and caregivers with receiving the most appropriate options and services to meet their complex healthcare needs.

Major duties include, but may not be limited to: Monitors delivery of obstetrical services. Coordinates Community Care for pregnant Veterans and/or CHAMPVA clients. Tracks maternal and fetal outcomes. Facilitates access to local VA and community services and resources. Navigates the coordination of maternity care throughout the local healthcare system. Collaborates with the Office of Community Care on consults to ensure seamless coordination of non-VA care.

Ensures pregnant Veterans and/or CHAMPVA clients receive a copy of the VA/DoD patient resource manual Pregnancy and Childbirth: A Goal-Oriented Guide to Prenatal Care. Educates pregnant Veterans and/or CHAMPVA clients about all VA Maternity Benefits, including newborn care, and reviews insurance options. Initiates, maintains, and deactivates pregnancy/lactation status in the electronic medical record.

Maintains a minimum of bi-monthly contact with the Veteran and/or CHAMPVA client being cared for by community providers. Documents correspondence in telephone encounter notes in the electronic medical record. Functions as a liaison between the Veteran and/or CHAMPVA client, contracted provider, and the local healthcare system.

Ensure Veterans and/or CHAMPVA clients resume VA healthcare within 12 weeks post-delivery (or sooner with certain comorbid conditions), and collect assessment data based on patient signs, symptoms, and history. Provides health care advice to patients and family members seeking advice, utilizing the nursing process and triage protocol and policies. - Develops a plan of care using information from the patient, family, and or provider.

Completes required documentation, assuring that it is reflective of patient needs. Is familiar with and knowledgeable about the disease process. Completes work assignments within established time frames; adapts to changes in or additions to work assignments including cross-covering other Women Veterans and/or CHAMPVA clients Program clinics whenever necessary; with necessary training and reprioritizes as necessary.

Maintains current emergency training and can instantly report responsibilities of the position in the event of an emergency. Report any problems with coworkers or patients or process issues to the Manager in a timely manner. Completes mandatory training, updates, memo/minutes updates, and annual training updates. The MCC RN provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's goals.

Participates in process improvement activities and huddles. Performs other duties as assigned by the supervisor and/or manager.
