The VA New Jersey Health Care System is currently seeking an Outpatient Registered Nurse to work Intermittently as a member of our Patient Aligned Care Team (PACT) at the Lyons, New Jersey VA campus. The PACT Outpatient Registered Nurse assesses, plans, implements, and evaluates care based on age specific components, coordinates care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) is responsible for providing competent, evidence-based gender-specific care to primary care patients and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting. They provide direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department.
The PACT RN will manage assigned panels, utilize registries, and ensure appropriate evaluation and access is provided to patient panels. This incumbent provides health promotion, health education and coaching on wellness, disease prevention, and chronic care management by assisting in and directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization.
Duties include but are not limited to: Demonstrates leadership in delivering and improving holistic, primary care through collaborative strategies with others. Evaluate practice in an ongoing process, based on best evidence, and provide peers with informal constructive feedback for improvement. Fosters a safe and supportive environment conducive to the professional development of healthcare professionals.
Administer medications and procedures per established policies and guidelines. Influence care outcomes by collaborating with members of the interdisciplinary team through active participation in the unit/program level quality improvement process and initiatives as well as customer service programs. Partner with others to effect change and produce optimal outcomes, and support colleagues through knowledge sharing to provide safe, quality outpatient nursing care.
