Make a lasting impact on the health and well-being of Veterans as an Outpatient Registered Nurse with the VA Black Hills Health Care System in Rapid City. In this role, you will build meaningful relationships with Veterans while helping them manage their health, prevent illness, and navigate primary and specialty care services.
Working alongside an interdisciplinary team, you will combine clinical nursing practice, education, care coordination, and health coaching to develop individualized plans that reflect each Veteran's needs and goals. This is an opportunity to strengthen your nursing practice while contributing to coordinated, Veteran-centered care across the continuum.
Duties include, but are not limited to: - Assess Veterans' health needs and develop individualized nursing plans of care based on clinical findings and the Veteran's goals. - Deliver and evaluate outpatient nursing care across primary and specialty care settings to support safe, effective, and coordinated treatment. - Coordinate care across the continuum to help Veterans access appropriate services and maintain continuity among members of the interdisciplinary care team.
- Educate and coach Veterans, families, and caregivers on wellness, disease prevention, treatment plans, and strategies for successful self-management. - Support Veterans with chronic conditions by providing ongoing nursing care, education, and guidance designed to promote optimal health outcomes. - Administer medications, treatments, and nursing procedures in accordance with established clinical practices and individualized care needs.
- Collaborate with interdisciplinary team members to develop, implement, and evaluate coordinated approaches to Veteran care. - Apply evidence-based nursing practices and participate in activities that improve the quality, effectiveness, and efficiency of outpatient care.
