Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU The incumbent is a GS-12 Social Worker, Program Coordinator who functions as the COMPACT Act Coordinator within the Addictive Disorders Treatment Program (ADTP) and Mental Health Clinic (MHC) interdisciplinary team at the William S. Middleton VA Medical Center in Madison WI. As COMPACT Act Coordinator incumbent has responsibility as the primary navigator for Veterans receiving care under COMPACT Act.
This includes coordinating transitions in care for Veterans receiving acute suicide care under COMPACT Act between varying venues of care, types of care (medical and mental health) and location of care (direct care and community care) in collaboration with assigned care coordinators at each step of the Veteran journey.
The Social Worker Program Coordinator will be instrumental in providing connection and referral to assist with proper evaluation for eligibility in other programs and is the facility subject matter expert on the COMPACT Act. Incumbent facilitates and/or provides outreach, education and collaboration efforts with community agencies and is responsible for education to the local facility (leadership and providers) on COMPACT Act matters.
In addition to COMPACT Act Coordination duties, incumbent for this position provides a full range of social work services within the addictive disorders treatment program (ADTP), outpatient mental health clinic including triage work (same day access), individual/group psychotherapy, crisis intervention, assessment and treatment of complex psychosocial problems resulting from mental illness, chronic and acute illness, senescence, or socioeconomic and family concerns.
Social Worker Program Coordinator will spend 50-75% of their time delivering clinical care to patients and may be cross trained in other mental health-social work departments in order to provide coverage to ensure patient care & operations. Required: LCSW, Provide clinical supervision for licensure (Social Work) Preferred: Substance Use Disorders (SUD) Work Schedule: Monday through Friday, 8:00am - 4:30pm.
