The Clinical Dietitian is the medical nutrition expert responsible for independent planning, developing and directing all nutritional care activities for assigned clients, including those with complex medical and nutritional needs. He/she supports the facility's mission and coordinates nutritional care with other health care providers, the client, the client's family and the community.
Duties include but are not limited to: Conducts a comprehensive nutrition assessment for lower acuity patients at nutritional risk using the Nutritional Care Process (NCP), including nutrition focused physical examinations, to establish a nutrition diagnosis and determine risk and/or degree of malnutrition. Develops and implements Medical Nutrition Therapy (MNT) to address the nutrition diagnosis, evaluates and monitors the effectiveness and outcomes of MNT interventions.
Modifies the plan as needed to achieve positive patient outcomes. Documents MNT in the medical record using the Assessment, Diagnosis, Intervention, Monitoring, Evaluation (ADIME) format. Assesses educational needs and provides nutrition counseling for individuals and groups, incorporating a learning approach that considers the cultural, religious, educational, and medical situation of the patient.
Includes information regarding budgeting, menu planning, food purchasing and preparation, food storage and sanitation. Provides appropriate follow-up in accordance with patient's treatment goals, referring patients for outpatient counseling, community or home health service as appropriate.
Documents in the medical record, the education materials provided, the care plan determined by the provider and the client, an assessment of the client's comprehension level to the education given and the expected compliance. Maintains productivity standards and practices, effective time management, and prioritizing tasks. Assists in achieving compliance with regulatory agency standards, including state, federal, and JC standards.
Advises interdisciplinary health care team of nutritional recommendations, goals, and efficacy of nutrition therapy. Discusses pertinent findings and interprets nutritional information which will assist the team in developing comprehensive patient treatment plans.
