The position is located in the Ambulatory Care Clinic within the Division of Nursing at Crownpoint Service Unit. The facility provides primary care clinic/outpatient services to beneficiaries in the surrounding community. The primary purpose of this position is to serve as a Patient Care Coordinator/ Case Manager for the outpatient clinical services, including the satellite clinics. A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021).

Coordinate comprehensive patient care by collaborating with physicians, nurses, social workers, physical therapists, case managers, public health, and social services to address patient needs, manage clinic and follow-up visits, maintain health care requirements, and achieve timely, cost-effective outcomes.
Facilitate patient care conferences, provide interpretation support when applicable, conduct chart reviews, and document care coordination activities to monitor effectiveness, continuity of care, and compliance with documentation standards. Collects relevant data for program evaluation, including service utilization, identified problems, and the status of resolutions.
Assesses and monitors patient satisfaction to support program improvement and enhance the quality and effectiveness of care coordination services.
