Job Summary

RN for the Hospice/Short Stay Community Living Center to deliver fundamental, knowledge-based care to assigned clients. The role involves assessing, planning, implementing, and evaluating patient care tailored to age-specific needs, with a focus on patient education, self-management, and satisfaction.

The RN will administer medications and procedures according to policies, collaborate with the interdisciplinary team to improve care outcomes and take responsibility for individual nursing practice

Key Responsibilities

Major duties include but are not limited to: Demonstrate integration of biopsychosocial concepts, cognitive skills, and clinical competence in patient care. Assess, plan, implement, and evaluate individualized care based on age-specific needs. Coordinate care focused on patient education, self-management, and enhancing customer satisfaction throughout the continuum. Administer medications and procedures following established policies, guidelines, and safety protocols.

Collaborate with interdisciplinary team members to improve care outcomes and promote safe transitions. Provide evidence-based, competent nursing care to residents while supervising licensed vocational nurses and nursing assistants. Assist in directing nursing education, orientation, competencies, and quality improvement initiatives. Partner with residents, families, and team members to ensure effective and safe care transitions across healthcare settings.

Apply foundational critical thinking and theoretical knowledge to clinical situations, seeking guidance as needed. Ensure accurate documentation and compliance by consulting with the MDS Coordinator for RUG score accuracy. Support quality improvement efforts by utilizing evidence-based practices and outcomes data. Maintain accountability for delivering safe, comprehensive, and patient-centered nursing care within the community living center.

EDRP Authorized: This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment.

Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply. Permanent Change of Station (PCS): Not Authorized. PCS Appraised Value Offer (AVO): Not Authorized.
