Job Summary

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment.

Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Total Rewards of a Allied Health Professional The BHIP-PCMM Social Worker Care Coordinator is in the Mental Health Behavioral Sciences (MHBS) service line. The incumbent functions as a coordinator for care of Veterans engaged in Mental Health (MH) services at this facility working with a team focused on MH-Care Coordination.

The Social Worker will provide support to the MH Care Coordination Clinical Team in their role as Mental Health Treatment Coordinator (MHTC). Major duties include, but are not limited to: Perform clinical management and tracking of Veterans engaged in MH services as identified by PCMM, care coordination, and case coordination. Assess treatment needs and engagement, evaluate if at risk and coordinate care with MH programs.

Track, contact, and refer Veterans identified under PCMM to include approximately 1,000 Veterans. Participate in committees and workgroups related to patient safety, suicide risk, and engagement in treatment services. Participate in the development and implementation of Standard Operating Procedures for the MHTC portion of the BHIP Program supporting MHBS service line. Conduct telephone and VVC interactions with Veterans assigned to MHBS, psychoeducation of Veterans (e.g.

resources and programs), supportive counseling of Veterans and care coordination services to assigned Veterans consistent with the goals and treatment methods of MH. Provide supportive Care Coordination to increase access and enable fluid transition between programs and resources with a focus on strengthening the Veteran's ability to address acute and chronic problems. Manage a diverse Veteran caseload with some Veterans needing more intense follow up than others.

Potential engagement with Community Care providers. Performs other duties as assigned. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade). Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year).

Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience. Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.

Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
