This Supervisory Social Worker (Outpatient Subspecialty) Social Work Navigator is at the VA Boston Healthcare Systems located at the Jamaica Plain location. This position is a full-time 40 hours per week.
The purpose of this position is to serve as a Supervisory Social Worker (Outpatient Subspecialty/Social Work Navigator) at the VA Boston Healthcare Systems, Jamaica Plain Location. In the Supervisory role, the incumbent ensures high-quality, coordinated psychosocial and care navigation services for Veterans with complex specialty care needs, providing direct clinical and administrative supervision for up to five (5) GS 9 - GS 12 Social Worker.
In the Social Work Navigator role, the incumbent provides a complex array of services, both clinical and administrative, for non-VA Boston Veteran's receiving specialty surgical care at VA Boston. Duties include, but are not limited to: Provides clinical and administrative supervision to Social Work staff, including support for independent licensure and performance management.
Oversees staffing and workload distribution, setting priorities and monitoring service delivery and documentation quality. Collaborates with interdisciplinary teams across multiple services to resolve complex psychosocial issues and support integrated care. Prepares reports, supports program evaluation, and contributes to continuous quality improvement efforts. Coordinates staff training needs, organizes onboarding, and provides consultation on population-specific needs.
Monitors Social Work services within assigned clinics, including consults, care coordination, productivity, and performance measures. Conducts targeted outreach throughout the VISN to assess needs and clinic capacity and strengthen Veteran access to specialty and surgical care. Provides individualized care planning and case management throughout the peri-procedural continuum to ensure all psychosocial and practical needs are met.
Develops and supports data-driven quality improvement processes to evaluate program effectiveness. Identifies gaps in services and works with teams to develop solutions that best meet Veterans' needs. Work Schedule: Monday-Friday, 8:00 a.m. - 4:30 p.m. Telework: Ad-Hoc only. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade). Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year).
Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child. Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA.
