This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment.
Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
The Psychosocial Rehabilitation and Recovery Center (PRRC) is a distinct ambulatory program providing a therapeutic, supportive learning environment for Veterans with the goal of maximizing functioning across life domains and promoting meaningful community roles. Participation intensity decreases as Veterans acquire skills; discharge is mutually determined by the Veteran in treatment and the PRRC team.
The Senior Social Worker supports PRRC by providing intensive case management, psychotherapy (including evidence-based psychotherapies), and care coordination to ensure continuity of care and sustained stabilization. The incumbent serves as an expert on community mental health resources and functions as a liaison to connect Veterans to supports within VA and the community, in both individual and group settings.
Clinical Assessment & Treatment Planning Independently complete comprehensive psychosocial assessments; obtain input from the interdisciplinary team, the Veteran, and when available the family/caregiver/significant others. Conduct intake/brief psychosocial assessments, evaluating needs, engagement willingness, and care coordination options.
Develop behaviorally written treatment plans with measurable objectives and concrete interventions; include goals for psychosocial clinical treatment. Monitor e-consults, complete assessments, and present recommendations to the PRRC treatment team. Update treatment plans in accordance with VA policy timelines.
Direct Clinical Care Provide ongoing individual and group psychotherapy, including evidence-based modalities in which the incumbent is trained/certified (e.g., CBT, DBT, ACT, MI, CPT, PE). Deliver short-term, solution-focused psychotherapy; facilitate family services and psychoeducation groups/classes (mental health, substance use, recovery, behavioral interventions).
Provide specialized treatment for Veterans with serious mental illness (SMI) and dual diagnoses (e.g., co-occurring substance use disorder). Capture caseload activity/workload via electronic encounters per Mental Health Executive Leadership timelines. Case Management & Recovery Support Maintain a caseload of PRRC Veterans; serve as Recovery Buddy, delivering case management, individualized treatment planning, and reintegration support.
Participate in and manage community-based group processes; may transport Veterans to community outings that build engagement and recovery skills. Apply counseling approaches (e.g., conflict resolution, anger management, healthy coping) and advanced practice judgment regarding self-care capacity, family support, and treatment needs. Suicide Prevention & Risk Management Independently identify high-risk Veterans and provide crisis intervention.
Complete C-SSRS, CSRE, and Safety Plans per the VANTHCS Suicide Prevention SOP, meeting documentation timelines. Provide follow-up for no-show Veterans per local procedures; consult with the Suicide Prevention Coordinator, Suicide Risk Reduction Committee, or supervisor as needed. Provide comprehensive assessments and collaborative safety plans upon initiation of the High Risk for Potential Suicide List, in compliance with VA policy.
Care Coordination & Community Partnerships Develop and maintain effective partnerships with VANTHCS and community programs (mental health, substance use, co-morbid conditions) to obtain resources and ensure continuity of care. Serve as liaison to connect Veterans to VA/community services; build strong community relationships to support successful care transitions. Keep Clinical Reminders current for assigned Veterans.
Compliance, Quality, and Performance Improvement Develop and maintain treatment/documentation standards meeting Joint Commission and CARF expectations for Substance Use Disorder and PRRC services. Implement methods for measuring effectiveness of social work practice (e.g., outcome evaluations, EBPs) for medically and psychiatrically complex Veterans.
Education, Training, and Program Support Provide orientation, coaching, in-service training, and continuing education for social work staff and interdisciplinary members. Supervise social work students (with supervisor approval); develop training plans/goals, assess progress, and provide reports to field liaisons; participate in the school affiliation and field instructor training processes.
