The incumbent is a Social Worker assigned to a North Florida/South Georgia Veterans Health System (NF/SGVHS) VA Community Living Center (CLC), located in Lake City FL and serve as a float position to other areas for an unspecified period of time within Social Work Service, to include but not limited to Primary Care, Emergency Department, and Medicine/Surgery.
The Social Worker's responsibilities include the assessment of mental health and psychosocial problems for Veterans admitted for short term rehabilitation or long term care. Clinical work includes the provision of assessment, goal oriented treatment planning, discharge planning, crisis intervention, short-term solution-focused or problem solving focused evidence based interventions, support groups, and counseling/psychotherapy (appropriate to level of licensure).
**The Social Worker may be required to drive and/or travel using personal vehicle or government vehicle to other sites of care within the NF/SGVHS, as well as, to community partners, as necessary to accomplish work assigned. While travel would be rare, when it is expected, it would be at least 50% of work week.
Major Duties include but are not limited to: Utilizes evidence-based and best practice approaches to assist Veterans, caregivers, and staff in managing behavioral factors associated with chronic medical problems and in promoting medical compliance. Provides ongoing team consultation services to other social work section team members.
Work is conducted in collaboration with interdisciplinary treatment teams and with Veterans and/or their family members/significant others/caregivers. Independently conducts psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, educational and other diversified backgrounds. The Social Worker has knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e.
acute, chronic, and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan. Provides individual, group, and family evidence based psychotherapies and treatment (i.e. PST, MI, MET, CPT, PE, CBT, Seeking Safety, etc.) appropriate to level of licensure to Veterans and their families/caregivers. Evidence based therapeutic interventions are reviewed regularly during clinical supervision.
Responsible for planning and organizing work, coordinating with clinical interdisciplinary team members, other professional and clinical staff, and management personnel. Responsible for developing, updating and using current social worker resources within VA and community social service programs to ensure that Veterans are receiving education, information and referrals to current and viable resources to meet their needs.
Responsible for educating the Veteran and family members/caregivers on the available services and assisting them in establishing the appropriate referrals based on the Veteran's preference or that of his surrogate decision maker. Able to engage in positive and professional customer service interactions with stakeholders at all levels. Ability to communicate effectively both orally and in writing.

Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may NOT be substituted for the master's degree in social work.
